We regret that Peak Family Medicine and Urgent Care in Gunnison will be permanently closed after Friday, September 23rd. We would like to let all of our patients know that we can still provide care remotely from our Montrose location via video calls if you decide to keep your care with us. If you decide to move to another medical office your records are available to almost all medical offices on the western slope via Quality Health Network. Let your new medical office know they can retrieve your records via QHN. If the office does not have QHN access we can fax your records to them upon request. We can also mail your records to your home but this requires additional processing time and will incur a printing and mailing cost or CD burning cost if you prefer a digital copy. Thank you for being apart of our family. We wish you the best and hope to care for you in the future if the need arises.
